A front-end MySQL database viewing tool such as PHPMyAdmin is great for creation of databases and other database administration. If you are not familiar with the PHPMyAdmin, visit the site, download it and deploy it on your web server. It will save you time, greatly.
Assuming you now have PHPMyAdmin, login to the PHPMyAdmin system. There will be an option to create a new database on the main page, the "Databases" menu.
As an alternative to PHPMyAdmin, the documentation MySQL command line examples to create a user and database for PHP Live! will provide a step-by-step instructions to create a MySQL database from the command line.
